Medication review for the elder in Region Skåne



Medication contributes to a better health, but it presents a challenge when used incorrectly. Adverse reactions of medicines cause a high percentage of hospital admissions for elderly. Common causes include lack of consensus and understanding of the prescribed treatment, and errors involved in the transition between different types of care.

Therefore, health care authorities in Region Skåne (Sweden) are implementing a “medication review” before elders are discharged from hospitals.

The goal for 2012 is that 30% of all patients over 75 years old discharged by internal medicine departments and with more than five types of prescribed medicines will receive a medication review.

This review is conducted by clinical pharmacists, who analyze individual patients’ medicine prescriptions and propose appropriate changes.

The aim of this initiative is to achieve consistency between prescribed and used medicines.

The regional public health strategy for the period 2010 - 2013 contemplates the work elderly people as a priority.
